Birth Certificates
- There is a $23 fee to obtain a certified copy of a birth certificate (short-form or long-form.)
- Only the person, mother, father, brother, sister, spouse, or grandparents may request a copy of a birth certificate.
- The person requesting the copy must complete a form and have proper identification.
- In-person payment methods: Cash or credit cards only (no personal checks accepted).
Requests By Mail
- When requesting a copy by mail, a notarized proof of identification and affidavit is required.
- Mail order payment methods: Money order or cashier’s check only.
Death Certificates
- There is a $21 fee to obtain a single certified copy of a death certificate. (Additional certified copies of the same death certificate may be purchased at the same time for $4 each.)
- Only the mother, father, son, daughter, brother, sister, spouse, grandparents, or grandchildren may request a copy of a death certificate.
- The person requesting the copy must complete a form and have proper identification.
- In-person payment methods: Cash or credit cards only (no personal checks accepted).
